We ask you to please fill in the information below, which will be sent directly to us. Please note that we need to have this information a minimum of 14 days prior to your departure date to ensure that your requests can be made.

NOTE: Your passport must be valid for a minimum of six months after the date of your arrival home.

Please note here if you have any special needs that we will need to inform the flight, dive or boat company of. For instance: allergies, physical hinders, meal preferences (vegetarian)

NOTE: Liveaboard trips require that each dive pair has an SMB and that all divers must have their own dive computer.

NOTE: Please list your height (cm), weight (kg), shoe size and if you are normally small (S), medium (M), large (L) or extra large (XL)
